i've been cleaning out file cabinets at my new job and i found this old form. it was used to ask businesses who had been listed as a trading reference what their experience was with the customer.
its so much easier to decide with pictures, isn't it? my favorite is "slow and unsatisfactory" holding the gun to his own head!
